Personalized Medicine Approach Provides More Benefit for Patients with High Cholesterol than Current Guidelines

Statins are a class of drugs that lower cholesterol and thereby reduce the risk of heart disease and stroke. They work by preventing the synthesis of low-density lipoprotein (LDL or “bad cholesterol”) in the liver and promoting its clearance from the blood. They are the most effective cholesterol-lowering drugs currently available and are the cornerstone of the National Heart, Lung, and Blood Institute’s National Cholesterol Education Program (NCEP) treatment guidelines.

The NCEP recommends a “treat-to-target” strategy, in which patients are given specific statin doses to achieve a desired level of LDL cholesterol in the blood. In this case, low LDL cholesterol is the “target.” Yet some physicians are questioning whether treating to any target is the best approach to fighting disease. A recent study in the Annals of Internal Medicine suggests that “tailored treatment”, an approach attempts to practice personalized medicine by estimating three factors, is more effective than a treat-to-target strategy [1].

Healthy People is consortium of diverse, motivated and dedicated agencies and organizations committed to working together to help achieve health goals and objectives for the nation. As a national initiative, Healthy People 2020’s success depends on public input.

Every 10 years, the HHS leverages scientific insights and lessons learned from the past decade, along with knowledge of new and emerging issues, data, trends and innovations to set the nation’s health priorities. In 2007 and 2008, the Healthy People 2020 framework was established, consisting of the vision, mission and overarching goals. In 2009, specific objectives and strategies to achieve them were developed. This month, the HHS is requesting public comments on the draft set of proposed objectives for Healthy People 2020. You are invited to comment on the proposed objectives and topic areas and suggest additional objectives and/or topic areas that you feel are missing.

New Genes Associated with Blood Pressure and Hypertension

High blood pressure or hypertension affects more than one in three people worldwide and is a major cause of strokes, heart attacks and heart failure [1]. The degree with which blood pressure traits can be inherited suggests a genetic component. However, limited consistent evidence of genes associated with blood pressure have been produced. A new study in the journal Nature Genetics reports for the first time a number of genes showing significant associations with blood pressure and hypertension across the genome [2].

Although large-scale genome-wide association studies (GWAS) have been used successfully to identify genes associated with common diseases and traits, studies on blood pressure or hypertension have failed to identify loci at a genome-wide significant threshold (p-value < 5 x 10-8). The significance of GWAS data relies on several variables, including the accuracy of phenotypic measures, density of markers and size of the study population. Thus, if blood pressure variation in the general population is due to multiple genetic factors with small effects, a very large sample size is needed to identify them.
